Participant Liability Waiver — Sasquatch Ice Sports, LLC · James Sewell Ice Rink, Cloudcroft, NM

Assumption of Risk. I am aware that ice skating, figure skating, hockey, stick & puck, broomball, and/or all ice skating programs and activities involve inherent risks, dangers, and hazards which can result in serious personal injury or death. I am also aware that the ice skating rink and surrounding areas contain dangers that can cause serious injury or death. I hereby freely agree to assume and accept all known and unknown risks of injury arising out of ice skating, hockey, broomball, and/or all rink activities.

Release and Waiver of Claims Agreement. For allowing me (or the participant I register) to participate in public skating, figure skating, hockey, stick & puck, broomball, and/or all ice skating programs and activities at the James Sewell Ice Rink, I agree, to the fullest extent permitted by law: 1) to waive all claims that I have against Sasquatch Ice Sports, LLC, the James Sewell Ice Rink, and their owners, operators, and respective affiliates, agents, representatives, and employees arising out of public skating, figure skating, hockey, stick & puck, broomball, and/or all ice skating programs; and 2) to release Sasquatch Ice Sports, LLC, the James Sewell Ice Rink, and their owners, operators, and respective affiliates, agents, representatives, and employees from all liability for any loss, damage, injury, or expense that I (or my next of kin, parents, guardian, or estate) may suffer as a result of participation, including as a result of negligence or breach of contract in the operation, supervision, design, or maintenance of the facility.

Minors. If I am registering a minor, I represent that I am the minor's parent or legal guardian and I make these agreements on the minor's behalf as well as my own. I agree to follow posted rink rules and staff instructions. For hockey programs this also includes the USA Hockey Code of Conduct, USA Hockey Neck Guard acknowledgment, and Half-Shield Waiver.
